Our Hospice skilled nurses, RNs and LVNs, provide compassionate, specialized care for patients with terminal illnesses, focusing on comfort and quality of life. Our nurses assess and manage symptoms such as pain, shortness of breath, nausea, and anxiety, ensuring the patient remains as comfortable as possible. They also administer medications, provide wound care, and coordinate with our medical director, social worker and other healthcare professionals to develop personalized care plans. Hospice nurses educate patients and their families about the disease process and what to expect, offering emotional support and guidance throughout the end-of-life journey.

Personal Care
From our Certified Nurses Aides,personal care focuses on ensuring the comfort and dignity of the patients during the final stages of life. These services include assistance with daily activities such as bathing, grooming, dressing and toileting which can become challenging for patients with serious illness. The goal is to maintain your loved one's hygiene and well-being while promoting a sense of dignity and comfort. Caregivers may also help with repositioning the person to prevent bedsores and offer emotional support during these sensitive moments.

Bereavement
Yellow Rose Bereavement Services provide crucial emotional support to families after the death of a loved one. These services begin at the start of hospice service and continue months after your loved ones' passing, recognizing that grief is a long-term process. Our Hospice Bereavement Counselor offers one-on-one sessions, group support meetings and educational resources to help families navigate their feelings of loss. They may also provide assistance with practical matters, such as memorial planning or connecting with community support groups, ensuring that family members are not alone in their grief journey. The primary goal of Hospice Bereavement Services is to help survivors cope with the emotional and psychological impacts of losing a loved one.

Spiritual Support
Our Spiritual Support and Chaplain Services focus on addressing the emotional and spiritual needs of both patients and their families during the end-of-life journey. Chaplains, who are trained in various religions and spiritual traditions, offer a compassionate presence and provide opportunity for patients to explore their beliefs, fears and hopes. They help individuals reflect on their lives, offer prayer, or facilitate rituals that may bring comfort, regardless of religious background. Chaplains also support families in navigating complex emotions, offering guidance on finding peace and meaning at the end of life's journey.

Licensed Master Social Work
Our Social Worker plays a vital role in supporting both patients and their families through emotional, social and practical challenges that arise during the end-of-life process. She serves as a resource for families, helping them navigate complex decisions related to care, finances, and legal matters, such as advanced directives or funeral planning. Our Social Worker also helps patients and families manage the emotional weight of facing a terminal diagnosis, offering counseling and providing a safe space for individuals to express their feelings of fear, grief, and uncertainty. She works closely with the rest of the hospice team to ensure that the emotional and social needs of the patient are addressed in a comprehensive, compassionate manner.
